MAHEC is seeking candidates to facilitate the development, implementation and ongoing optimization of a multi-disciplinary care coordination program that serves patients and care teams of the Family Medicine and OB/GYN divisions.  This position will also directly supervise nursing and allied health staff working in specific roles assigned to the program.

Specific responsibilities include, but are not limited to:

  • Provide technical consultation to MAHEC regarding care coordination best practices based on prior knowledge and experience as well as regular learning and literature review.
  • Oversee the hiring and orientation process for care coordination program staff.
  • Foster a patient-centered culture and model attitudes and behaviors that support learning, innovation, and data-driven decision making.
  • Provide direct patient care as part of the process of developing new programs and to ensure appropriate continuity of care.
  • Ensure the development, implementation and ongoing maintenance of care coordination staff training and professional development program that builds staff capacity to grow with changing program needs.
  • Work collaboratively with divisional leadership to develop, track and respond to program budget and financial performance with the goal of program fiscal sustainability.

Required Qualifications:

Any combination of education and experience equivalent to:

 

  • Bachelor of Science in Nursing and 10 years of recent clinical nursing experience, including a minimum of 5 years of ambulatory care management; AND
  • 2 years of experience with care coordination/care management program management and supervision required.
  • Current licensure as a Registered Nurse with the North Carolina Board of Nursing or a compact state as defined in North Carolina Board of Nursing.
  • Current BCLS required.
  • Masters of Science in Nursing preferred.
